Did you know that you can book well visits, same day sick visits (SOON TO BE FLU CLINICS!) through your MyChart account?

If you have not yet signed up for MyChart, you can call us at 978-975-3355 during normal business hours or obtain a code the next time you are in the office.   You can obtain a MyChart activation code at your next visit, or over the telephone. It requires additional information such as (your) date of birth and contact information.  If you prefer to complete the registration on your own time, simply print off a SignUpForm and drop it off or send it in to our office.

MyChart, enables you to view your child’s medical record, including notes from medical visits and lab tests, get past immunization history, view and pay your bill, and even to download camp and sports forms.  Parental access differs depending on the patient.  Patients 13 years of age and older are eligible for their own account.

  • On your phone, download the MyChart mobile app from your application center.  
  • Open the MyChart app, select your country and state, and pick “PPOC” (see logo bottom right) from the listed organizations.
  • Log in!

At age 18 the patient will have full control of their MyChart account and the parent/legal guardian’s access will be automatically discontinued unless Adult Proxy Access has been approved by the patient.
The following services are available to individuals with Pediatric and Adolescent Proxy Access:
 Ability to review, make or cancel appointments
 Ability to view portions of the patient’s medical record such as allergies, medications, histories, problem list and recent visit information
 Ability to receive certain lab results
 Ability to communicate with providers via secure messaging regarding health concerns
 Ability to request prescription refills
 Links to other sites that will provide health information

When the patient turns 13 years old, access is automatically transitioned to limit the services to which the parent/legal guardian has access. 

Only the following services are available to parents/legal guardians once the patient turns 13:
 Ability to make appointments
 Ability to communicate with providers via secure messaging regarding health concerns
 Ability to view immunization histories
 Ability to view growth charts
 Ability to pay bills
 Links to other sites that will provide health information

Consent to Access Developmentally Challenged Patient Record
Requests for MyChart access for patients over the age of 13 who are developmentally delayed or cannot consent to granting access to a parent/guardian will be reviewed by the patient’s primary care provider or practice confidentiality officer. The proxy will receive complete (clinical and non-clinical) access.
